What is Pediatric dentistry (Paedodontics)?
Pediatric dentistry is the branch of dentistry that deals with children’s teeth. This dentistry focuses on a wide spectrum of conditions affecting dental development in children. Most common pediatric dental services include fillings, extractions, pulpotomy, pulpectomy, space maintainer and preventative treatments.

When we do detect cavities in children, our priority is to restore the health of their teeth with a restoration that’s strong, safe and long lasting.
There are various types of tooth fillings that can be used on your child. Where possible, we use a tooth-coloured filling material called composite resin when restoring children’s teeth. This is also commonly used for adult tooth fillings.This white tooth filling can be used on deciduous, baby teeth and adult, permanent teeth. We prefer to use it because:
- Functionally, in comparison to some temporary materials it is stronger
- It does not require as much tooth preparation as amalgam (metal) fillings
- Aesthetically, it is as close as you can get to the colour and translucency of natural teeth
- Composite resin is also suitable as fissure sealants for children’s permanent molar teeth – to reduce the risk of decay and the future need for fillings
- Composite resin, however, is not ideal for every child’s tooth or cavity as this material works best as small fillings in low-stress areas.

Pulpotomy or Pulpectomy for Children

The pulp of the tooth is the hollow inner core of the tooth. The pulp contains nerves, blood vessels, connective tissue, and reparative cells. The purpose of pulp therapy in pediatric dentistry is to maintain the integrity of the affected tooth, so that your child’s tooth is not lost, until his/her permanent tooth is ready to erupt.
Dental caries (cavities) and traumatic injury are the two main reasons for your child’s tooth to require pulp therapy. pulp therapy is often referred to as a “nerve treatment,” “baby or children’s root canal,” “pulpotomy,” or “pulpectomy.”
The two common forms of pulp therapy in children’s teeth are a pulpotomy and a pulpectomy.
A pulpotomy is a dental procedure that is performed when the decay in a child’s tooth reaches into the pulp (nerve) tissue. The infected part of the nerve tissue within the crown portion of the tooth is removed to prevent further inflammation and spread of disease (caries). Next, a sedative material is placed within the tooth to prevent bacterial growth and to calm the remaining nerve tissue.
After the pulpotomy is finished, your child’s tooth is restored with a stainless steel crown (SSC) on the back molar teeth to re-establish normal chewing function and to continue to hold the space until the permanent tooth can take its place. On the upper front teeth, either a regular stainless steel crown or a white esthetic stainless steel crown can be placed. This is performed in one visit and causes no more discomfort than placing a routine filling.
A pulpectomy is required when the entire pulp is involved. During this treatment, the diseased pulp tissue is completely removed from both the crown and the roots. The canals are cleansed, disinfected, and in the case of primary teeth filled with a resorbable material. Then a final restoration is placed which would be the same choices as for a pulpotomy on a primary tooth. A permanent tooth would be filled with a non-resorbing material and either a temporary stainless steel crown or an adult ceramic, porcelain, or gold permanent crown would by placed.

Why Remove Baby Teeth When They Fall Out Anyway?
- Eight of the baby molars are replaced between 10 and 12 years of age, and it’s these teeth which are commonly decayed. If the patient is younger, we don’t wait for them to fall out because decay progresses quickly, which can lead to pain and infection in a short period of time. If these are infected, there’s a risk that the infection can damage the developing adult tooth beneath.
- You need to protect your child’s other Healthy Tooth’s. By leaving a decayed or infected tooth in the mouth, you’re also leaving bacteria to spread to other teeth, which has a snowball effect on the child’s decay rates.
- Not all decayed or infected teeth can be fixed reliably, so they’re removed to achieve a successful and stable long-term outcome.

Space Maintainer for Children

What is a Space Maintainer?
If a child has lost a baby tooth too early, a space maintainer can help keep the gap open until the adult tooth comes through. Space maintainer is a small, stainless steel band and a loop device is attached to one of the teeth next to the space. It holds the gap open and prevents the neighboring teeth from shifting until the permanent tooth erupts and comes into place.
Usually a space maintainer (also called a teeth spacer) is a fixed attachment used for a single missing tooth. If your child has several missing baby teeth, our paediatric dentists will discuss with you the most suitable type of space maintainer for your child’s individual situation.
Why your Child may need a Space Maintainer?
A space maintainer may be necessary when a primary tooth (baby tooth) has been lost prematurely. Some reasons for premature tooth loss include:

What to expect after your child gets a tooth space maintainer?
Most children do not experience any pain following the fitting of a space maintainer. We ask the patients (children) to avoid eating hard or chewy foods such as ice cubes and chewy lollies as these can dislodge the space maintainer. The space maintainer is removed when the permanent tooth starts to erupt into the space.
